What is GUIDE?

The GUIDE Program is a free Medicare-supported service that provides care coordination, caregiver support, and help with everyday needs to help those living with dementia remain in their homes and communities.  If you have noticed memory loss, confusion, or other changes in thinking, we can help you explore next steps, including evaluation and diagnosis.

What type of support does GUIDE provide?

  • A care navigator to help coordinate care and connect to local community resources
  • Education, training, and support for family caregivers
  • Up to $2500 per year in respite care, giving eligible family caregivers a break from time to time
  •  A personalized care plan that fits each person’s goals
  • 24/7 helpline for ongoing support and life
  • Medication support and safety checks
  • Regular check-ins and ongoing monitoring
  • Support for transitions and referrals to specialists

Who is Eligible?

  • Must be diagnosed with dementia
  • Must have Medicare Parts A & B as primary payer (cannot have Medicare Advantage)
  • Must live at home or in an assisted-living community (not nursing home or long-term care)
  • Cannot be enrolled in hospice
